The town of Westwood’s Council on Aging is an engaging, active and welcoming environment that serves the diverse needs of approximately 4,500 citizens 60 and older, as well as disabled adults. The COA is seeking a compassionate, respectful and resourceful Social Worker to provide outreach, advocacy and support to the town’s senior residents.
Qualifications: master’s in social work or human services; Mass. licensure in a counseling discipline, e.g., LICSW, LMHC, Psy.D.; three or more years of experience in outreach, gerontology or counseling.
Representative duties: performs crisis intervention by phone or in person at the COA or through home visits. Conducts assessment of clients; determines case management plans; coordinates and implements delivery of services. Makes home visits, assists seniors in defining needs, facilitates access to services and makes referrals. Identifies resources available to seniors for preventative efforts. Maintains confidential records in accordance with federal and state regulations. Logs daily phone calls and activities; maintains monthly statistics and written reports. Assists all Westwood residents with applications for fuel assistance and SNAP benefits; assists clients with applications for community resources. Complies with mandated reporter guidelines and procedures and reports information to the appropriate Protective Services Agency. Communicates with client families, agencies and other organizations as situation dictates. Works with COA Elder Nurse and Public Health Nurse to assist with client needs.
Salary: $67,248-$83,984 DOQ. Robust benefits package and generous time off allowances.
