Outreach Coordinator – Southborough Senior Center

About the Role:
The Outreach Coordinator will be a vital resource for seniors and their families, offering information, support and advocacy to help older adults maintain their independence and quality of life. You will work directly with seniors, caregivers, service providers, peers and other community stakeholders to address needs and connect individuals with appropriate services.

Key Responsibilities:

  • Conduct outreach and assessments to identify seniors’ needs and connect them with appropriate services and resources.
  • Offer supportive guidance to seniors and their families, promoting self-determination, dignity, and respect.
  • Collaborate with local agencies, town departments, and regional partners to expand resources and referral networks.
  • Support caregivers by providing guidance, information and compassionate referrals.
  • Assist in developing programs that support aging in place, in addition to contributing to public education efforts, presentations, newsletters, and community outreach.

What We’re Looking For:

  • A dedicated professional with a background in human or social services; LSW preferred.
  • Bachelor’s degree in human services, social work or a related field; several years of relevant experience required.
  • Solid understanding of social work principles, including confidentiality, boundaries and respect for self-determination.  This position is a mandated reporter in accordance with law
  • Familiarity with the challenges seniors face and the local, state and federal resources available to support them.
  • Strong written and verbal communication skills, with the ability to build trusting relationships with seniors, families, service providers, and colleagues.
  • A compassionate, empathetic, resourceful and solutions-oriented mindset.
  • A current MA driver’s license is required
